Procedures for Submitting an Incident/Injury Report

1. Below are instructions on how to complete the online Incident/Injury Report. 
   When you are ready, please CLICK HERE to enter the report.

   In order to complete the coach report of incident/injury you must have the following information:

   1. Player name, address, jersey number and player ID number

   2. Parent name and email

   3. If incident, opposing player’s jersey number and player ID number (if available and applicable)

   4. Details of what occurred to cause incident/injury

   5. Coach report of incident/injury must be completed by a league official (coach, team manager, referee, etc.) and not the parent

    

   Please note the following:

1. Electronic copy of completed report will be forwarded to the parent and GSSA. All reports are reviewed by GSSA.

2. Injury reports are approved and sent to Bollinger Insurance for processing.

3. Incident reports are sent to GYSA D&P committee for review.

4. Bollinger Insurance will contact parent, via email, with login information to complete insurance claim form.
